Webinar Objectives

  • Discuss the current landscape of home-based primary care, learnings from recent research regarding the prevalence and health and social care needs of high-need, homebound older adults, and care models serving this population.
  • Identify top barriers and facilitators to scaling home-based primary care from the payor and provider perspectives.
  • Explore opportunities for health policy leaders to support a home-based medical care ecosystem aimed at improving access and health outcomes for high-need, homebound older adults. This includes building on CMS’s advanced primary care efforts to better address the needs of an increasingly aging population.

The webinar is supported by The John A. Hartford Foundation and is part our project, Leveraging Policy Reforms to Scale Home-Based Primary Care.
