- Discuss the current landscape of home-based primary care, learnings from recent research regarding the prevalence and health and social care needs of high-need, homebound older adults, and care models serving this population.
- Identify top barriers and facilitators to scaling home-based primary care from the payor and provider perspectives.
- Explore opportunities for health policy leaders to support a home-based medical care ecosystem aimed at improving access and health outcomes for high-need, homebound older adults. This includes building on CMS’s advanced primary care efforts to better address the needs of an increasingly aging population.
