Emma Tepfer, of the Aetna Healthiest Cities and Counties team, presented a poster project at the APHA Annual Meeting in Atlanta, GA. The poster outlines ongoing work in Durham County that uses CHWs integrated into interdisciplinary teams to achieve the triple aim. Specifically, this presentation looked at a model for CHW integration that has generated promising results in the management of community-dwelling adults with diabetes type II and hypertension.
